Each entry: {"name":"slug","label":"…","type":"text|date|select","default":"…","options":[…]}. The placeholder in the DOCX must match the name exactly, e.g. {{contractor_name}}.

Replace DOCX (optional)

Leave blank to keep default_independent_contractor_agreement.docx. Edit it locally in Word first — keep placeholder spelling intact, and don't format mid-token (it splits the placeholder into multiple runs and breaks rendering).
